Submit a Student Absence Notification
If the student is physically unable to submit the form, someone else may submit the request on their behalf. In the event a student is hospitalized for any reason, the student is encouraged to make an appointment with a Student Assistance Services staff member to discuss their return to classes. Student Assistance Services can ensure follow-up services are provided or assist with student withdrawal if needed.
PLEASE NOTE: Issuing a Student Absence Notification does not "excuse" the absence, nor serves as a request for the absence to be excused, regardless of the reason. Instructors have the authority to excuse absences based on the documentation you provide. For more information about excused absences, please refer to the course syllabus, speak directly with the instructor, or review Student Rule 7.
